• Copy of CNIC
• The Form C must be duly signed from a Medical Officer 

Procedure:

After fulfilment of the above requirements, the applicant is required to visit : Additional Deputy Commissioner Office Lower Chitral. Where the picture of the applicant along with other data will be stored in the system and the learning certificate will be issued to the applicant.

After expiration of the learning certificate, the applicant is required to submit the certificate in the office again and then the applicant will be called for a driving test.

The applicant who passes the driving test will then be issued a Computerized Driving License.
